Availment Procedures

OUTPATIENT AVAILMENT PROCEDURE
  1. For Consultation
    • Secure an Eastwest Healthcare Consultation/Referral Form from the Eastwest Healthcare Coordinator, HMO Office/Concierge, or Industrial Office of the Eastwest Healthcare-accredited hospital/clinic by presenting your Eastwest Healthcare membership card, supported by any government-issued ID.
    • At the Accredited Doctor’s room/clinic, present the Consultation/Referral Form along with your membership card and proceed with the consultation.
    • If your condition requires consultation with another specialist, appropriate referral will be made by the Accredited Doctor or Eastwest Healthcare Coordinator. You have to follow the same steps for consultation to secure an Eastwest Healthcare Consultation/Referral Form.

  2. For Laboratory or Diagnostic Procedures
    • Present your Eastwest Healthcare membership card, supported by any government-issued ID to the Eastwest Healthcare Coordinator, HMO Office/Concierge, or Industrial Office of the Eastwest Healthcare-accredited hospital/clinic.
    • The attending medical staff will call Eastwest Healthcare’s Customer Service for verification of eligibility and coverage. Once verified and approved, an Eastwest Healthcare Diagnostic Request Form will be issued.
    • Present the Diagnostic Request Form to avail of the laboratory or diagnostic procedure.
IN-PATIENT AVAILMENT PROCEDURE
  • An admitting order must be secured from Eastwest Healthcare-accredited Doctor or Attending Physician, whether admission is elective or an emergency case leading to confinement.
  • Present the admitting order, along with your Eastwest Healthcare membership card and government-issued ID to the Admitting Section of anyEastwest Healthcare-accredited hospital. The attending medical staff will call Eastwest Healthcare’s Customer Service for verification of eligibility and benefit plan. The member will then be tagged as admitted. The hospital staff will facilitate admission.
  • Once coverage is determined, a Letter of Authorization (LOA) will be issued to the hospital where the member is admitted.
  • Member is required prompt filing of his/her PhilHealth Sickness Benefit Claim prior to hospital discharge.
EMERGENCY CARE AVAILMENT PROCEDURE
  • At the Emergency Room of any Eastwest Healthcare-accredited Hospital, present your Eastwest Healthcare membership card, supported by any government-issued ID (after necessary medical treatment has been given for emergent cases).
  • The ER staff will call Eastwest Healthcare’s Customer Service for approval of availment.
  • Sign bills upon discharge.
  • For ER cases leading to confinement, follow the inpatient availment procedure.
Dental Availment Procedure
  • Refer to the Eastwest Healthcare Provider Booklet and check for the list of Affiliated Dentists or dental clinics. You may also contact Eastwest Healthcare’s Customer Service for assistance.
  • Call the accredited dental clinic to confirm schedule – different clinics have their own set of scheduling their patients, some may accept walk-in and some are by appointment.
  • Once confirmed, proceed to the dental clinic on your scheduled appointment. Present your Eastwest Healthcare membership card, supported by any government-issued ID. The attending staff will call Eastwest Healthcare’s customer service for validation of eligibility and coverage.
  • Before signing the Eastwest Healthcare Dental Treatment Form at the dental clinic, check if the dental procedures/services listed are correct.

COVID 19 UPDATES MARCH 2021

On March 13, 2021, the Department of Health (DOH) confirmed the detection of a COVID-19 variant, called P3, which was found in local samples in the Philippines. Viruses naturally mutate because, just like humans, they also need to undergo structural changes for them to adapt to their host body. Viruses that would have the same mutations will form a variant.

CERVICAL CANCER AWARENESS MONTH of - May 2021

Cervical Cancer

Ito ang ikalawang pangkaraniwang uri ng kanser ng babae sa Pilipinas. Tinatayang mayroon ng 7277 na bagong kaso at 3807 ang namamatay dahil dito kada taon.

 

Walang sintomas sa umpisa

Madalas walang napapansing sintomas ang mga babaeng may cervical cancer. Lumalabas ang sintomas nito kapag malala na ang sakit.

Dengue Awareness - Month June 2021

Dengue

Ang dengue ay ang pinakamabilis na lumalaganap na vector borne disease sa buong mundo. Ito ay dulot ng Dengue Virus na merong 4 na uri o serotype (DENV1, DENV2, DENV3, DENV4).

 

Aedes aegypti at Aedes albopictus

Ang mga lamok na Aedes aegypti at Aedes albopictus ay tinutukoy na vector sa paglaganap ng dengue. Ang babaeng lamok ay nagdadala ng virus sa nakakagat nitong mga tao. Kadalasang makikita ang mga lamok na ito sa madaling araw o dapithapon sa mga lugar na mayroong tubig na di umaapaw. Maaring matukoy ang mga lamok na ito dahil sa mga kulay puting marka sa katawan at paa nito.

Nutrition Month - July 2021

Sa buwan ng Hulyo, nutrisyon ang pangunahing pokus para sa ating kalusugan. Sa ika-47 na
Nutrition Month, tuunan natin ng pansin ang tamang nutrisyon para sa pamilya lalo na sa mga anak.

Mayroong 3 uri ng malnutrisyon:

  • Undernutrition
  • Overnutrition
  • Micronutrient Deficiency

Ito ay nagdudulot ng kakulangan (wasting) o labis (obesity) na timbang, kakulangan sa tangkad (stunting) at marami pang ibang sakit.

Asthma Awareness - Month August 2021

Ang ikalawang linggo ng Agosto ay Asthma Awareness week.

 

Ang asthma o hika ay ang pamamaga ng daanan ng hangin sa baga. Ito ay nagdudulot ng mahirap na pagdaloy ng hangin sa baga. Dahil dito, maaring maranasan ang mga sumusunod:

 

  • Wheezing o huni sa paghinga
  • Ubo
  • Paninikip ng dibdib
  • Pagkapos ng hininga
